How We Help

Home safety modifications

Sometimes all a person needs to live independently is a few home modifications, such as ramps and grab bars. We can help provide these modifications at no cost to you.

Resources for caregivers

Helping someone manage their daily activities is demanding both physically and emotionally. We can connect you with resources to get a break, cope with stress, plan for long-term care, navigate Medicare and more.

If you're caring for a loved one with dementia, specialized services are available to assist you in addressing your unique needs and challenges. Learn about our REACH program.

Advocates for nursing home residents

An ombudsman is a trained staff member or volunteer who advocates for quality of life and care for people living in nursing homes and assisted living facilities. After moving into a nursing home or assisted living facility, a resident may need help to continue a life of dignity, respect, choice and as much independence as possible.

Medical benefits counseling

We can help you understand your options for Medicare, prescription coverage, long-term care, advance directives and more – for free.
