The Access & Assistance (case
management) program helps eligible adults, age 60 and
over, and their caregivers obtain temporary services needed to help older adults remain
in their own homes.
Eligibility
Tarrant County residents who are 60 years
old or over. In addition, a client must be
in danger of being placed in a nursing home,
or have experienced a recent health care
crisis, or have two or more unmet activities
of daily living.
Once eligibility
is determined, a home visit by an Access & Assistance case manager will be
made to determine a person's needs and ability to carry out normal daily
activities necessary for independent living.
Family members are encouraged to participate
in the assessment. A care plan,
reviewed every 90 days, will be developed by
the case manager, based on the needs of the
client.
There is no fee for this temporary service, but
contributions are accepted.
Services include
• Home health services, such as personal assistance
• Homemaker services to assist with activities of daily living
•
Benefit counseling, including assistance in
filling out applications
for benefits
• Transportation to medical appointments
• Home-delivered meals
Other services may be arranged as
needed. Some services will
require a client contribution based upon
ability to pay.
Advocacy
The case manager will advocate
for the client to receive benefits and
services provided by the community and problem solve between
the client and family
members.
Professional Referrals
We encourage appropriate referrals from
hospital discharge planners, home health
social workers and other gerontological
professionals.
