ACCESS & ASSISTANCE (CASE MANAGEMENT)

The Access & Assistance (case management) program helps eligible adults, age 60 and over, and their caregivers obtain temporary services needed to help older adults remain in their own homes.

Eligibility

Tarrant County residents who are 60 years old or over. In addition, a client must be in danger of being placed in a nursing home, or have experienced a recent health care crisis, or have two or more unmet activities of daily living.

Once eligibility is determined, a home visit by an Access & Assistance case manager will be made to determine a person's needs and ability to carry out normal daily activities necessary for independent living.
